Job Description:
Position ConceptAssist with the Single Point of Access (SPOA)
process for the Community Based Services. Serves as liaison
to/central point of contact for provider staff, community
residential and mental health providers, and Department of Children
and Families to ensure accurate and timely performance of mandated
functions required to access mental health services and medicaid
eficiency for children in the custody of the Department.Essential
Functions* Coordinate with Nurse Care Coordinator as necessary to
carry out assigned duties and resolve issues with Medicaid
enrollment, etc.* Responsible for screening and securing
therapeutic placement for children in out of home care. * For
children in the Department's custody, provides consultation to CBC
project staff in assessing mental health screening, professional
assessment and timely, quality treatment at levels appropriate to
the severity of children's conditions.* Monitors the level of care
for children in out-of-home placement, serving as a resource for
transition of levels. Serves as a member of the team to assess
levels of care.* Provides technical assistance to CBC project staff
to ensure quality of services (i.e., timeliness, appropriate and
effective referrals to mental health services in the community).*
Maintains DCF and Eckerd databases to track timeliness and
completeness of service provision.* Meets all DCF requirements for
reporting and service coordination, including required monthly
reports on demographics, units of service, status and timeliness,
assuring that reports meet quality standards and that all reporting
is completed in a timely and accurate manner as required by policy
and contract.* Evaluates the services being provided through the
Mental Health Services for Children in the Custody of the
Department process, as well as serving as a member of the quality
assurance/quality improvement initiatives team to continually
evaluate effectiveness.* Provides training to CBC project staff
regarding the SPOA process and related work flow issues.* Develops
and reviews the comprehensive policies and procedures manual and
quality improvement program.* Develops tracking systems to report
data collection and statistical analysis related to assigned
programs.* Attends staff development training for professional
growth.* Designs, produces and/or contributes to monthly, quarterly
and annual reports as required and specifically regarding the
duties and responsibilities listed above.* Address plans, issues
and tasks related to OHC and provide recommendations to the
Management Team..* Coordinates on-going training on the functions
of the SPOA team to partnering agencies.* Monitors to ensure that
children in custody are referred for Comprehensive Behavioral
Health Assessments (CBHA), through on-going communications with
staff and therapeutic agencies.* Ensures communication and
information sharing between case managers, counselors, caregivers,
and service providers.* Facilitates monthly multi-disciplinary team
(MDT) meetings between Eckerd and therapeutic providers to discuss
progress and barriers to achieving admissions and discharges.*
Monitors and reviews case plans to ensure that all recommendations
from CBHAs are being included and implemented.Other Duties Include,
But Not Limited To* Participate in training and development
activities as required.* Report any acts, incidents or conditions
that reflect the possibility of inappropriate youth-to-youth or
staff-to-youth relationships.* Report any use of physical force and
all unusual incidents per Eckerd policy and state guidelines.*
Perform other duties as assigned.Position SpecificationsEducation:
Bachelor's degree, from an accredited college or university, in the
areas of psychology, social work, or related human service field
required.Experience: Three years experience working with troubled
youth and their families and/or mental health services, including
administrative responsibility for compliance required.Skills:
Ability to work with detailed information, analyze data, recognize
established patterns and exceptions, and create effective
corrective action plans to bring exceptions back into compliance
with established requirement. Ability to create and deliver
effective training modules, using effective public speaking skills.
Detail oriented, excellent verbal and written communication skills.
Ability to interpret, adapt and apply guidelines and
procedures.Other: Must meet state criminal background check
requirements.Travel: Must be able to meet requirements for Eckerd's
Auto Insurance and be able to drive for business purposes. Must
maintain an appropriate and valid state driver's license.Physical
Demands: Ability to sit at desk for long periods of time, and
tolerate extended periods of data entry. Must be free of
communicable diseases as required by the state and to the extent
that knowledge is attainable under the law.
