Eckerd Connects Community Alternatives, Case Management A Program of Eckerd Connects and governed by a community-based board, Eckerd Connects case management has developed an integrated and collaborative system of care for safety, well-being and permanency of vulnerable and at risk children, while strengthening and supporting families. In its role as a case management organization we provide service and oversight to 1000 children that require protective supervision.Please follow the link for more information about this program: https://eckerd.org/family-children-services/foster-and-adoption/eckerd-community-alternatives-pasco-pinellas/Position ConceptTo contribute to the outcomes and success of Eckerd Case Management by developing and delivering program quality improvement systems and programs designed to improve individual and organizational performance. This position will provide qualitative reviews of case files and support case management to bring files into compliance.Essential Functions* Partner in the development and monitoring of QI initiatives designed to advance and sustain program goals, measure therapeutic components, assess the capacity of program staff, and evaluate client outcomes.* Tracks and Supports Supervision and Staffing follow up activities to expedite permanency such as home studies, ICPC requests, parent contact, and family finding efforts to meet safe and timely permanency goals.* Reviewing of records to assist in assessment and decision making related to permanency.* This position will assist with any Request for Action identified as well as any steps needed for Corrective Action Plans.* QI will review, track, and submit Incident Reports identifying any follow up that needs to occur to mitigate risk. Follow up with providers who submit Incident Reports to generate a companion report if needed. * Complete case reviews as requested by Supervisor/APD/PD.* Files identified to be audited will be pre-audited by QI and results shared with CM/CMS/ADP.* Participates in Peer Review process.* Attend meetings at the direction of APD/PD relating to QI job description.* Attend Court as necessary.* Manage and ensure compliance with Title IV E Candidacy (Safety plan compliance).* Participates in on-call as Case Management Supervisor.Other Duties Include, But Not Limited To* Carry a restricted case load of 15 children or less.* Attend and participate in professional development training for professional growth as developed by the Agency/CBC, as required by the state of Florida and in accordance with Agency policy and procedures.* Report any acts, incidents or conditions that reflect the possibility of inappropriate youth-to-youth or staff-to-youth relationships.* Report any use of physical force and all incidents per Eckerd Connects policy and state guidelines.* Perform other duties as assigned.* This position reports to the Quality & Permanency Assistant DirectorPosition SpecificationsEducation: Bachelor's Degree required from an accredited university in the area of study in human services, criminology, juvenile justice, psychology, sociology, counseling, special education, education, human development, child development, family development, marriage and family therapy, and nursing. Preference will be given to candidates with a background in trauma informed care. Must complete the required minimum continuing education/training per CBC policy for continued certification.Certifications: Must be a fully certified child welfare professional. Florida Certification Board Child Welfare certification required.Experience: Two Years of experience working with children and families involved in Dependency Court cases, and supervisory experience required. Three years of experience preferred. Skills: Ability to practice all Eckerd Connects/CBC policies and procedures related to privacy and security practices cited in the Health Insurance Portability and Privacy Act (HIPPA) applicable to my program and position; Excellent verbal and written communication skills; Public speaking skills; Must be highly motivated and self-directed. Knowledge of community resources, service referrals and linkage, and child development; including physical, mental, emotional and cultural and ethnicity issues required. Ability to work under stressful situations with poise. Must attend relevant skills-enhancement training and demonstrate required program specific knowledge. Must be computer literate. Detail and organizational skills critical. Time management skills necessary to accomplish this position. Other: Must have appropriate and valid state driver's license. Must meet state criminal background check requirements.Physical Demands: Ability to regularly lift/move up to 25 pounds and sit for long periods of time; Ability to work irregular schedule as required to complete work assignments successfully. Must have reliable transportation & ability to use personal car to transport clients. There may be potential exposure to clients that may exhibit physically and/or verbally aggressive behaviors. There may be potential exposure to communicable diseases. Be familiar with the facility's fire prevention/first aid equipment, evacuation routes, exposure control and disaster plans. Maintain client and staff contact information and evacuation plan in the event of a natural disaster and must have availability for contact for self during a natural disaster.
