Job Description:
HCA Healthcare is seeking a dedicated RN Case Manager for our
Inpatient Rehabilitation Facility (IRF) team. This position
requires a compassionate individual focused on patient-centered
care, responsible for assessing, planning, implementing, and
coordinating patient care from admission to discharge. The role is
pivotal in the hospital setting, ensuring that care is timely,
efficient, and professional, aligning with our commitment to
high-quality healthcare services. The RN Case Manager will be a key
player in our interdisciplinary team, working across various units
to enhance patient outcomes and service delivery. Conduct
comprehensive patient assessments to determine required care and
resources. Develop and monitor individualized care plans in
collaboration with healthcare teams. Advocate for patient needs and
coordinate services across multiple healthcare providers. Evaluate
patient progress and orchestrate timely adjustments to care plans.
Educate patients and families on post-discharge care and resources.
Ensure compliance with healthcare regulations and standards. Strong
clinical skills in a hospital setting Excellent communication and
interpersonal skills Proficiency in case management software and
EMR systems Ability to coordinate and prioritize care effectively
Knowledge of healthcare regulations and accreditation standards

401k Current Registered Nurse (RN) license in the state of practice
Certified Case Manager (CCM) preferred
