Job Description & Requirements - Specialty: Physical Therapist - Discipline: Therapy - Duration: 20 weeks - 40 hours per week - Shift: 8 hours, days, nights - Employment Type: Travel Job Title: Home Health Physical Therapist Shift: Day Shift – 5x8 Hours (8:00 AM – 5:00 PM) Job Type: Contract / Travel Setting: Skilled Home Health Job Summary We are seeking an experienced Home Health Physical Therapist to provide skilled in-home therapy services to a diverse patient population. The therapist will perform comprehensive assessments, develop and implement individualized care plans, and deliver high-quality rehabilitation services in a home care setting. This role requires strong clinical skills, independent time management, and experience working with OASIS documentation. Key Responsibilities - Perform patient assessments and evaluations in the home setting - Develop and implement individualized treatment plans - Provide therapy for patients with orthopedic, neurological, cardiopulmonary, and post-surgical conditions - Educate patients and families on home exercise programs and safety - Document visits and progress accurately in EMR systems - Identify and report signs/symptoms of physical, psychological, or sexual abuse - Ensure patient safety and injury prevention - Complete billing and productivity documentation - Maintain compliance with Medicare and insurance guidelines Patient Population - Med/Surg - Cardiac, CHF, COPD - Ortho (hips, knees, shoulders) - Post-surgical - CVA, Parkinson’s - Trauma, amputees (AKA/BKA) - Respiratory, trachs - Falls, weakness, failure to thrive - ADLs and mobility training Productivity - 6–8 skilled home visits per day - Minimum of 6 productivity points/day - Self-scheduled visits based on office needs - 1–2 weekends per contract Required Skills - Home Health Physical Therapy experience - OASIS documentation (preferred) - Assessment & treatment planning - Cardiopulmonary & circulatory rehab - Ortho & Neuro rehab - Patient & family education - Safety & injury prevention - Equipment usage: walkers, wheelchairs, gait belts, oxygen, lifts - Strong clinical documentation Certifications & Requirements - Active State PT License - BLS Certification - Minimum 1 year experience - Valid driver’s license reliable car - COVID vaccination (or exemption) - Flu shot (or declination with mask compliance) Provided Equipment - Laptop - MiFi internet access - Portable printer - BayCare scheduling & documentation systems About LanceSoft Established in 2000, LanceSoft is a Certified MBE and Woman-Owned organization.
