Job Description:
HCA Healthcare, a premier provider of patient-centered
healthcare services, is seeking a dedicated Physical Therapist to
join our Home Health team. In this role, you will have the
opportunity to provide one-on-one therapeutic care in patient
homes, implementing customized treatment plans that enhance patient
mobility, reduce pain, and improve overall health. This position is
ideal for someone passionate about delivering personalized care and
making a positive impact on patients lives within their own
communities. Assess patient condition by observing physical and
psychological responses to develop effective treatment plans.
Implement physical therapy interventions to improve mobility and
alleviate pain based on patient-specific assessments. Educate
patients and families on exercises and in-home therapy solutions to
ensure a successful recovery. Regularly review and adjust treatment
plans based on patient progress and feedback. Collaborate with
other healthcare professionals to ensure comprehensive care.
Document patient care services by charting in patient and
department records. Expertise in physical therapy techniques and
treatments specific to home health care. Strong interpersonal and
communication skills. Ability to work independently and manage
visits to multiple patients’ homes reliably. Proficiency in using
healthcare databases and patient management software. Medical

Valid Physical Therapy license in your practicing state. CPR
certified.
