Role: $7500 SIGN ON BONUS!

The RN, Home Health provides direct skilled care nursing services to patients in the home setting. The RN also coordinates care and services for a designated group of patients in the role of case manager. The RN supervises the Licensed Practical Nurse staff and Home Health Aide staff as required by regulation and policy.

Qualifications: --- Current Florida license as RN, current active single-state out-of-state nurse license, or current active multistate/compact nurse license --- Minimum one (1) year of in-patient nursing, home health or hospice experience for external candidates; for internal applicants, minimum of 6-months of LPN experience with hospice/home health within an affiliate --- Maintains current CPR certification --- Previous experience working with an EMR/EHR (Electronic Medical/Health Record) system --- Mobile Driver - Valid driver's license and automobile insurance per Company policy --- Reliable transportation to meet visit schedule --- Ability to use equipment with visual and auditory mechanisms --- Ability to effectively communicate in English (verbal and written) --- Ability to manage the demands/stress that are associated with the job and /or the work environment, so that acceptable levels of performance and overall contribution can be maintained --- Self-directed and able to work with minimal supervision --- Able to display professional interaction with physicians, patients, family members, and staff --- Ability to physically access patient homes (i.e., navigate stairs and narrow spaces, tolerate heat and lack of air conditioning, unsanitary conditions, smoke, pets, allergens, or other hazards that may pose a safety risk) within any part of the service area --- Ability to perform the essential functions and physical requirements (including, but not limited to: lifting patients and/or equipment, bending, pushing/pulling, kneeling) of the job with or without reasonable accommodation --- Ability to demonstrate adequate knowledge of clinical situation unique to providing services to the homebound elderly population --- Excellent observation, problem solving, and nursing skills per competency checklist Competencies: --- Satisfactorily complete competency requirements for this position. Job Responsibilities: --- Performs clinical assessment on a continual basis to determine patient status and progress towards treatment goals; determines home care needs. --- Coordinates the Plan of Care, ensuring that an individualized Plan of Care is developed that accurately reflects the patient's evolving needs. --- Provides skilled nursing services to patients as indicated by physician orders and the Plan of Care, to include treatments and diagnostic procedures (therapeutic, preventive, and rehabilitative nursing actions). --- Provides counseling to the patient and family relative to nursing needs including patient safety. --- Responsible for the day-to-day coordination of clinical activities for an assigned group of patients, to include care planning, goal setting and physician liaison. --- Reports changes in the patient's condition to appropriate health professionals. Makes adjustments to the Plan of Care as instructed. --- Communicates accurately and completely to Physicians, staff members, patients, families, and supervisors; utilizes positive approaches when working with others. Maintains accurate timely documentation of all clinical activities. --- Adheres to principles of infection control including standard precautions for all patients. Identifies signs and symptoms of infection, documents and notifies Physician of any adverse findings. --- Supervises patient care given by other nursing team members (i.e., LPNs, Home Health Aides, and contract nurses). --- Performs other duties as assigned.
