Job Description:
Summary: The individual in this role provides oversight for the
health center under the CenterPlace Health scope and is responsible
for all operations at their assigned location. Responsibilities
include, but are not limited to, management of primary health care
providers, clinical support staff, and clerical support staff. This
role assists/directs patients with questions and inquires and
supports the health center to provide quality and effective care.
Additionally, this individual, works collaboratively with other
program managers at the site to assure quality, comprehensive care
is available to the population. The Center Manager assists the
Director of Operations in motivating the employees, monitoring
patient flow, training employees, and facilitating on-going
communications and collaboration with the administrative team.
Essential Functions: Provides oversite of health center site
activities in accordance with professional/industry standards and
CenterPlace Health policies: Plans, directs, and organizes work,
including staff scheduling. Recruits, interviews, and selects new
staff. Participates in orienting and training of new staff.
Evaluates staff by observing performance, client outcomes and
reviewing documentation of care/services. Provides counseling and
coaching for staff using a problem-solving approach Provides
instruction/guidance to staff on policy, procedures, and processes.
Conducts and participates in regular staff meetings. Manages the
activities of the day by staff, patients, students, and providers
to assure a smooth, efficient patient flow through the units.
Assures that required medical equipment and supplies are readily
available at the time it is needed, and in emergencies to include
medications, lab processing and notification of emergency medical
services when indicated. Observes infection control and standard
precautions. Works with clinical staff to accurately monitor all
clinical documentation and logs for accuracy and completion. Plans,
promotes, and coordinates all site services, including but not
limited to, medical, dental, and behavioral health services. Works
directly with the Field Operations Manager to manage the daily
operations of the center. Works collaboratively with dental and
behavioral health leadership to assure patient care standards are
met. Works collaboratively with the Chief Medical Officer and/or
his designee to ensure appropriate provider coverage for all
services. Analyzes patient flow problems and assists in instituting
corrective actions. Monitors staff performance in compliance with
established timeframes. Regularly interfaces with providers and
staff to facilitate communication and improvement activities.
Functions as EHR super user working closely with AthenaHealth to
ensure that appropriate training, error monitoring and corrections
are completed in a timely manner. Makes recommendations regarding
transfers, promotions, and terminations. Coordinates with PSR
Manager and Field Operations Manager to assure competencies.
Provides direction and support for staff to achieve organizational
goals and objectives. Participates as needed as a member of various
committees related to the overall improvement of the corporation.
Complies with company policies and procedures Promotes and complies
with NCQA Patient Centered Medical Home standards of care. Assures
that all documentation in the EHR is completed as required. Attends
all mandatory meetings and in-services.Utilizes best practices in
management to provide efficient and effective services that meet
organizational customer service, internal processes, and financial
goals: Involves staff in planning and evaluation of services. Works
collaboratively with other program managers at the site to assure
quality, comprehensive care is available to the population.
Collaborates/coordinates with key community partners to develop and
sustain systems, services, partnerships, and infrastructure as
established by the Bureau of Primary Health Care for Federally
Qualified Community Health Centers. Provides information and
feedback on center activities/services and acts as an advocate for
underserved populations throughout the community. Actively
participates in centralized and site level Performance Improvement
initiatives. Promotes team performance. Serves as a role model,
coach, and mentor for staff. Assists the Operational Leadership
Team: Assist with development of short and long-term goals and
objectives. Assists with development and management of the site and
organizational budget and business plan. Assists with the
development and evaluation of policies and procedures. Collaborates
with professional organizations, schools of nursing, physicians and
others as required in promoting the delivery of effective and
efficient clinical services. Attends and participates in staff
development and in-service education programs. Provides
administrative support, as needed. Other Supports clinical staff
and patients in referral process. Completes Daily/Day end
activities as assigned. Completes all required training and attends
meetings as assigned. Protects/observes patient confidentiality per
policies and procedures. Translates, as needed, if employee is
bilingual. Recognizes and responds appropriately to violent/abusive
situations, bomb threats, fire, and emergency situations. Provides
backfill for staffing as needed based on current training,
certification and/or licensure if appropriate. Understands Health
Information practices and assists when necessary. Understands the
referral process and assists when necessary. Minimum
Qualifications:
(Experience/Skills/Abilities/Education/Certifications): Must have
current LPN license. At least five (5) years nursing/MA experience.
Two or more years in a supervisory position. Current CPR
Certification. Strong verbal and written communication skills.
Demonstrated knowledge and ability to use Office 365 programs
including Outlook, Word, Power Point and Excel. Preferred
Qualifications:
(Experience/Skills/Abilities/Education/Certifications): Experience
or basic understanding of immunizations, laboratory services, and
340B drug program is recommended. Preferred experience/knowledge in
primary care. Cultural diversity, performance improvement and
coding. Physical Demands: While performing the duties of this job
the employee is regularly required to stand Walk, often for
prolonged periods. Use hands to finger, handle, or feel. Reach with
hands and arms. Stoop, kneel, crouch, or crawl. See, speak, and
hear. The employee must regularly lift and/or move up to 10 pounds
and frequently lift and/or move up to 25 pounds. Work Environment:
General medical clinic/office environment.Some travel may be
